Liability Waiver

By signing this waiver and release of liability on my own behalf and the behalf of my child, I hereby voluntarily release and forever discharge CHG Photography, its owners, employees, agents, insurers from any and all legal or financial responsibility for any personal injury, disability, illness, damage, or medical expenses arising from or related to my child’s participation in Summer Camp.

Photo Release

I hereby grant permission for CHG Photography and its owners to utilize my child’s appearance, performance or voice in any and all manner and media for the purpose of promotion, reporting, or publication. I understand that no royalty, fee or any other compensation of any kind shall become payable to me.

Code of Conduct

I understand the importance of this being a safe environment physically and emotionally. I recognize if my child acts inappropriately or is not listening, CHG Photography can and will limit their use of cameras and other equipment and will call for early pick up if needed. In addition, I acknowledge there will be an additional fee for late pick ups. Starting at 12:05, $15 for every 5 minutes past.

Payments/Refunds

I understand I will receive an invoice from CHG Photography for the payment of $160 after I submit the registration. I will remit payment within 48 hours of receiving the invoice from CHG Photography. I acknowledge that if I cancel 2 weeks before the camp I will receive a full refund, minus a $30 cancellation fee. I accept that I will not receive a refund if cancellation is less than 14 days prior to camp.
